Address is at the top of the form.  Enclose the fee of $50 payable to 
The Aero Club of Canada.  If you require the sporting license quickly, 
please sent by express post and enclose an addressed prepaid express post 
envelope for quick return.

If you need a sporting license at the last minute-e.g. at registration of a competition,

Make sure that you have fill out HPAC membership (s/b current) and rating 
information.  If this is your first sporting license and you were not born 
in Canada, please see the note on the form and include photocopies of required 
documents with the application.
